Post-Acute Care Unit
The warm and inviting private rooms of the Lakeside Post-Acute Care Unit are designed to help people recovering from hospitalization or illness regain their strength. Ecumen Parmly LifePointes offers rehabilitative services through a state-of-the-art physical therapy center, a combination of skilled nursing care, and physical, occupational, speech and aquatic therapies. The peace and serenity of Parmly’s gardens, walking paths, trees, and views of lovely Green Lake provide the perfect setting for health and recovery.
Think you may need physical therapy?
Consult with your doctor. Tell your doctor what issues you have been having with your activities of daily living such as walking, dressing, or balance. Your doctor will help you determine if physical therapy is your best option. Once you have a written doctor's order, call the business office at Parmly LifePointes to discuss your insurance/payment information at 651-213-2748. Then call the Therapy Department to schedule the appointment at 651-257-7940. If you need transportation services, you can make your arrangements by calling 651-257-7327. You will need to arrive 30 minutes early to your first appointment at the Vitalize! Wellness Centre to fill out the admissions packet.
At Home Care of Ecumen Parmly LifePointes provides an array of services to support active aging for our customers in the place they want to be, in their own homes. Services range from professional nursing and home health aid to light housekeeping, meal preparation, running errands and grocery shopping. At Home Care offers specially trained Parmly home health aides, supervised by a registered nurse, to help you in your home or your loved one’s home. We provide friendly, reasonably-priced home health care services.
Package A -- Daily 15 minute wellness visit and medication reminder by a Home Health Aid.
Package B -- Daily 30 minutes of morning or evening companionship, personal care assistance and medication reminder. Weekly linen change, bathing assistance and light housekeeping by a Home Health Aide.
Package C -- Daily 30 minutes of morning or evening companionship and personal care assistance. Twice Daily medication reminder. Weekly linen change, bathing assistance and light housekeeping by a Home Health Aide.

Over the years, the greater Chisago Lakes community has made major contributions to the success and well being of the Ecumen Parmly LifePointes community. This essential relationship between friends and neighbors continues to this day and is seen in a variety of Parmly outreach programs — called Community Connections — which help seniors in the greater community to age well within their own homes.
Community Connections programs, such as transportation services, South Chisago Meals on Wheels, Day Club, Caregiver Coaches and At Home Care programs are available to assist people to live independently in their own homes. Other offerings include such programs as:
Reach For The Stars — an exercise program designed for seniors;
Support groups for those affected by Alzheimers and Parkinson’s;
and outpatient services including PTOT & Speech and Aquatic therapies.
Transit Plus -- Whether you live at one of Ecumen Parmly LifePointes residences or in the Chisago community, our Transit Plus service offers transportation for medical appointments, local shopping and other important errands. We also provide specialized transportation and senior taxi services within the greater community. The "Senior Shuttle" offers door-to-door service from your home to your destination and back.
Point Pleasant Heights tennants enjoy complimentary scheduled transportation for local shopping in the Chisago City and Forest lake area.

South Chisago Meals on Wheels -- Enlisting a network of community volunteers, Meals on Wheels serves southern Chisago County seniors living within a 15-mile radius of Ecumen Parmly LifePointes with a hot, nutritious home delivered meal. The program provides a noon meal Monday thru Friday.

Adult Day Club -- The Ecumen Parmly LifePointes' Adult Day Club provides the essential elements of socialization and enjoyable activities within a gathering of peers who share similar backgrounds and life experiences. The goal of the Adult Day Club of Ecumen Parmly LifePointes is to socialize, stimulate, and motivate program participants while offering respite for the at home caregiver.
The Day Club is available to Chisago and Washington County residents 18 years of age and older, who are physically and/or mentally disabled and living at home. Transportation to and from Adult Day Club is available through the Ecumen Parmly LifePointes’ Transit Plus program.
Day Club operates Monday - Friday, from 9am to 3pm (except major holidays), and includes continental breakfast, family style dinner, and coffee hour with snacks, often made by participants.
In addition to daily exercise, our planned programs include board games, trivia and memory games, crafts, baking, walking groups, picnics, outings, plays, popcorn and movies, music and sing-a-longs, holiday celebrations, and more. Chapel services Catholic communion, and bible study are available through the Parmly Chapel.
Our Adult Day Club follows a medical model using trained professional staff, with services including assistance with bathing, medications, and other personal needs. Club participants can also take advantage of the Parmly Beauty Shop and, with a doctor's order, the Parmly rehabilitation services for those in need of OT, PT, and speech therapy.

Choose from a one or two bedroom apartment. Two bedroom apartments require a two-person household. Garden plots are available for tenants with "green thumbs". In addition to on-site banking, church services, and an exercise program, transportation, health care services, and grocery delivery are available.
Vindauga View Assisted Living Apartments provide the extra support and services needed to make enjoying life a bit easier. The 25 studio, one and two bedroom apartments are up-to-date, convenient and comfortable. To help ensure an optimum quality of life along the journey, Ecumen Parmly LifePointes has developed Memory Bridges, a supportive array of housing options and services for individuals with memory loss -- and for their loved ones, too. Taking into account each individual's cognitive abilities, we "custom-fit" our services, housing offerings and specially trained professional staff to meet their needs through all stages of memory challenges. Our specially designed housing amenities are named in honor of prominent women in the history of Ecumen Parmly LifePointes.
Margaret's House is an ideal residential setting for individuals experiencing the early to mid stages of memory loss but who are able to complete activities of daily living with the assistance of a caregiver. Here, an innovative design supports daily activities in personal care suites — including a private bath — surrounded by beautifully appointed common spaces within a home-like environment. Margaret’s House also offers a Day Club for people affected by memory loss who are living at home or in a caregiver’s home. Services are designed to help individuals maintain their highest level of physical, emotional, social and psychological well being.
Isabelle's House serves residents with advanced memory loss who require more assistance in achieving their optimum lifestyle. It’s the ideal environment for people who do not require skilled nursing care, yet do require one-on-one assistance with some activities of daily living. A nostalgic design features destination points and activities throughout the building to encourage active participation throughout the day.
The Cottage opened in 2010 and is nestled in a beautiful tree filled area near the lake. The Cottages provides memory care services for early to mid stages of memory loss. This unique remodel of an existing town home complex is a very home-like setting decorated with a "cottage" feel for eight clients. The building features private rooms and a beautiful outdoor space for gardening, walking or taking in some fresh air. Services are available 24 hours per day and strive to keep the individual as independent as possible.
Martha's House presents a smaller, home-like setting within The Residence which is more conducive to higher care levels for people with memory loss who require advanced support.
Day Club is a unique opportunity at Ecumen Parmly LifePointes for persons experiencing memory loss who are living at home or in their caregivers’ home. Day Club participants enjoy health promotion and social activities; family members and friends appreciate knowing their loved one is experiencing compassionate support. The services are designed to enable participants to maintain their highest level of physical, emotional, social and psychological functioning, ultimately securing a sense of well being.
The Residence at Parmly offers the choice of private and semi-private rooms, in addition to around-the-clock skilled nursing care. Here we carry forward Parmly’s historic tradition of providing the most compassionate elderly care. Dedicated, friendly and professional employees enhance the home-like environment, fostering dignity and independence for the residents.
Our broad array of services also includes entertainment, social programs, worship services, pastoral care, and home cooked meals. And, in keeping with the concept of staying engaged in life, a key component of living at The Residence is the active involvement of family members and friends.
All of Parmly’s services are based on a foundation of Christian warmth and compassion. We offer ecumenical pastoral care and a campus chapel for clients and the community, as spiritual well being is one of the six areas of wellness we support and encourage through our services. Spiritual care is extended to residents or tenants and their families as well as to staff without religious discrimination. Our chaplain works cooperatively with local religious leaders to provide spiritual support in times of life transition, health crisis, and bereavement.
The variety of worship and spiritual opportunities available on the Parmly campus gives both residents and tenants broad choices. Everyone is welcome to attend and be a part of a worship or spiritual experience that fulfills their spiritual need. For those unable to attend in person, Parmly offers live telecast of spiritual services across the campus on Channel 10.
The Residence -- In the chapel at the Residence, ecumenical worship services are held every Sunday morning and Wednesday morning at 9:15 a.m. Sunday worship is more formal and includes Holy Communion on the 1st and 3rd Sunday of the month. All our worship services are televised on a closed circuit cable channel for viewing in rooms and housing units throughout the Parmly campus. One of our 24 area community congregational pastors leads the Wednesday morning chapel service, and the Parmly chaplain leads Sunday worship.
We also offer Roman Catholic communion and rosary each Thursday morning, and Catholic laity provide private communion for our residents as requested.
In addition, a prayer service is held Friday mornings. Other offerings include a Wednesday afternoon Bible study, monthly "Services of Remembrance," the annual "All Saints’ Day," Catholic anointing twice yearly and a spiritual group for less cognitive residents twice a week.
Point Pleasant Heights -- We offer worship services in the Cedar Room at 2:30 p.m. on the 1st, 2nd, 4th and 5th Monday of the month with a Holy Communion service every 2nd Monday. Coffee and fellowship follow each service. Each Wednesday morning there is a Bible Study at 10:00 a.m. led by the Chaplain (except the summer.)
Lakeview Apartments -- Several community pastors lead worship on the 1st and 3rd Mondays in the library room with a Holy Communion service held for the tenants every third Monday. We also offer a Bible study every Friday afternoon.
Vindauga View Apartments -- A Bible Study Group meets every Tuesday afternoon at 2:30 p.m. in the dining room. Many also attend both of our the worship services at the Parmly Chapel on Sundays and Wednesdays as it is a convenient walk through the connecting hallways.
Day Club -- Bible Study is offered each Thursday before lunch. Holy Communion and pastoral conversations are available upon request. Each Wednesday most folks arrive to attend morning chapel.
Margaret’s House -- Spiritual Group is conducted each Monday, and individual pastoral visits can be arranged by request. Residents also enjoy worship in the chapel.
